De DEA agent Max Payne wil, nadat zijn vrouw en kind zijn vermoord, wraak nemen op de schuldigen. Tijdens een zaak wordt Payne echter vals beschuldigd van moord en wordt hij op de hielen gezeten door zowel de politie als door de verantwoordelijke criminele organisatie. Hij wordt hierbij vergezeld door Mona Sax, omdat zij wraak wil op de moord van haar zus.
