Caro Diario is een driedelig dagboek van een rusteloze veertiger op inspektietocht door Italië. 1. Op de Vespa: Nanni Moretti doet verslag van zijn tochtjes door de buitenwijken van Rome per scooter. Hij vertelt over de architectuur van de wijken, filosofeert over film en eindigt in Ostia, de badplaats waar Pasolini werd vermoord. 2. Eilanden: Nanni Moretti bezoekt de Liparische Eilanden waar hij een rustig plekje zoekt om te kunnen schrijven. Zeurende kinderen, toeristen en een reisgenoot die verslaafd is aan tv doen hem terug op de vlucht slaan. 3. Dokters: Moretti brengt verslag uit van zijn reis langs dokters en kwakzalvers.
