The Toxic Avenger verijdelt gedeeltelijk een bloedbad op een school voor gehandicapten. Een explosie brengt Toxie naar een parallelle wereld waar het kwaad regeert, en Noxie, Toxie's kwaadaardige tegenhanger, belandt in het vredige Tromaville. Nu is het aan Toxie om terug te keren naar de echte wereld om een einde te maken aan Noxie's slechte daden.
Nee, helaas is Citizen Toxie: The Toxic Avenger IV nog niet beschikbaar op Netflix in Nederland.
