In een Zuid-Amerikaanse stad breekt een moderne variant van de pest uit. De stad wordt afgesloten door militairen. Dokter Bernard Rieux (William Hurt) besluit te blijven om de slachtoffers van de ziekte zo goed mogelijk te verzorgen. De pest zorgt voor een aantal wanhopige situaties, waaruit de machteloosheid van Rieux en zijn helpers zo af en toe pijnlijk zichtbaar wordt.
