Emanuelle is erop uit om haar zus te wreken, die zelfmoord pleegde nadat ze was ontsnapt aan haar sadistische minnaar Carlo. Dus ketent ze Carlo vast in haar kelder, geeft hem drugs en dwingt hem toe te kijken hoe ze seks heeft. Terwijl Carlo allerlei bloedige verschrikkingen en kannibalistische handelingen begint te hallucineren, besluit hij dat hij zich moet losmaken en Emanuelle moet vermoorden.
