Nu Jigsaw en zijn kompaan Amanda dood zijn, lijkt het spel afgelopen. Maar als een nieuw videobandje van Jigsaw opduikt, blijkt het spel nog lang niet aan zijn eind te zijn. Twee FBI-profilers proberen de hele puzzel op te lossen en speuren door Jigsaws overgebleven spullen. Maar als een SWAT-lid ontvoerd wordt en onderdeel van het spel blijkt krijgt hij negentig minuten om, behalve zijn eigen leven, het leven van een oude vriend te redden.
Nee, helaas is Saw IV nog niet beschikbaar op Netflix in Nederland.
